One at a time, disconnect both wires from each injector and insulate the terminal ends so the circuit will not short to ground on the effected bank (1-3 for P2146 or 4-6 for P2149). Clear the codes and start the engine, check if the same code resets. If an injector is disconnected and the code does not reset, replace that injector. If code still sets after doing that possible ECM. I have also seen spread terminals on the harness which plugs into the valve cover connector set this code. Clear the code then wiggle wiring harness to see if code sets. I have been able with a small pocket screw driver to bend tabs so tighter connection was made in the connector. Truck was brought to me from another shop with this code setting, they were unable to repair after doing exactly what you have done so far. Turned out it was the poor connections at the plugin.

I would look very closely at the connector and wiring for any chafing of harness wires and spread terminals. I have seen ECM set this code but usually code would go active at start up and not be intermittent as you described.
ok guys i swapped to injectors out with #4 and the code is still coming back.i looked at every wire i could get to ,they all look good.its still throwing code only when it is at operating temp, when u go to slow down or u can have it n park and rev engine and it will set code.
Have you replaced the fuel filter?
I had the same code pop up several times. I found that a clean filter will help prevent it. Of course if an injector or wire is actually bad, a filter wont help, but, on my truck, a dirty filter did cause the problem.

I've gotta bite the bullet with this problem too. So tomorrow I'll take off the fuel rail, valve cover, examine the wires around the gasket (and other locations) for poor connection and corrosion; pull the fuel tubes, and check out my #5 injector as my starting place. I hope it's a simple fix cause I'm a bit worried about my recent experience of having had to limp around in my truck with inefficient power - locally; and can't take the risk on a longer run right now til I fix the mis fire problem.

I have had the same problem on my 03. Ended up replacing all 6 injectors. Apparently I had one or more in group 2 with bad solenoids. Be careful where you buy your remans at if you go that route or you could be doing the same as I did..... Replace them TWICE.... Good luck!
