Hello

my 2012 dodge 3500 has a intermittent problem where the engine gets really
loud under load. sounds like the fan ?.
and also i remotly started it today and it was surging at idle for a brief period

Well I have a 12 that does the same things you're describing and has since new. When the fans come on it sounds like a c-130 taking off. The 12's are notorious for high coolant temps and the fans coming on quite frequently. Also, the surging at idle on mine is very common especially at high idle and when colder. These are all normal symptoms from what I was told.

I just came home took it for a drive it was doing it right away drove it about 4 miles and it started sounding normal

 

Could have a problem with the fan coming on prematurely unless you have the a/c on. You could turn the a/c off before you exit the truck next time and see if that helps. If I leave my a/c engaged when I shut the truck off then upon start up the belt will make a horrible sound as the compressor engages but goes away after a second. The fan should only come on at 217 degrees and above during normal operation like while towing up a hill or sitting in traffic heat soaking.

 

does the remote start not turn on the AC & stuff?

or does it just resume with ever setting you left the HVAC on?
