Depends on what you're wanting to do (tow, race, maximize mpg, etc.). You can adjust the POD settings with the unit plugged in and the truck sitting at a stop light or even running down the road (wouldn't recommend for obvious safety reasons), but you can't adjust the REVO settings - this is only done with the truck parked so you can cycle the key on and off to update your ECM. People are running with the Smarty plugged in all the time. You should be able to do a search and read up on what they're doing.

Personally, I just set everything up initially (REVO and POD), and then leave it until the next time I want to plug the Smarty in to put it back to stock or adjust the settings.

Minimum mods for any power adder should be guages, drop in air filter or better, replacement muffler or better...if going above 70ish hp then at least vb and tq conv for auto trans.....

any suggestions for a better muffler instead of straight pipes?

I only have experience with the cat back borla (was pretty much given to me below cost)...but none with stand alone muffler replacement....others will chime in on that one....from what i've seen, magnaflow and donaldson are the most popular...with an emphasis on donaldson...

Think the Donaldson is supposed to be for normal people (sounds good, not too loud, no drone)...open a post on the Donaldson Muffler and get people who have it to respond.
