assuming you got it enable from dealer first. Manual high goes like this 1- start truck 2- turn cruise control on 3- press the cruise set button (engine will idle up to about 1100rpm) 4- using the resume/coast button you can increase the rpms up to 1500rpm no need to touch the throttle. If it does not idle up after setting the cruise then you do not have it enable. If you touch or hold the brake it will not work.

Here is the original thread discussing this TSB.

On the '04.5s you only need to have the dealer use the OBD to enable your high idle feature.

Well, I have an 04.5 and the cruise control high idle works fine. Was installed with TSB 18-004-04. Then it has to be turned on by the dealer using the DRBIII. Then I had the TSB 18-037-04 done and the high idle did not work. Has to go back in and turn it on again.

I think that there are a number of break points in the 04.5 engine run. My engine was built 01-11-04 and I know of changes that were later than that. It may be a check and see if it is for what you have. My dealer used the DRBIII scan tool to read the computer and determine what can be done. He read the computer then got online with the scan tool to find out what TSB's that I could use. He then downloaded the flash and installed it in the truck. Other than that, I have no idea what can and cannot be used. All I know is that I have a 04.5 and the high idle works great. Note that the computer was read with the DRBIII and not the ODBII.

For awile the enable feature wouldn't work on the 04.5's, but all that has been corrected. You only need the DRB for the enable on the 04.5 on up. I have probably done close to a hundred of them now and will do more now that the snowbirds are starting to show up.

motorhead, you could be right about it just needing to be enabled, but when I got the invoice back from the dealer it definately stated that they performed TSB 18-004-04 at the my request.
