High Idle / Cold Start
Your truck should have it in the ECM already. It has to be pretty cold and about 6 conditions need to be met in order for it to operate. Try searching Cold start/high idle. There are some previous posts on here explaining everything.

Many don't come from the factory with it installed.
Easiest way to get it is to borrow a Smarty. The Smarty will reprogram the ECM with the High Idle and will leave it there after you return your CTD to stock, and return the Smarty.
Dealers charge around $35.00 to re flash the ECM. Start a post asking for someone nesar you....buy them lunch and make a new friend! Cheaper.
But you may have it already. Parameters are very precise and must be meet exactly.

If you have the 1200 rpm high idle....you also have the 3 cyl idle.
"Additionally, if Intake Manifold Temperature (IMT) is less than -9°C (15°F), and all of the parameters above are met, three of the cylinders will be shut off upon reaching 1200 rpm, creating a slight change in engine sound which is normal. This allows the engine to create increased heat in the cooling system, allowing more rapid engine warm up and cab heating."
