Injector Return Line

How is the washer on the return placed?
Is it on the bolt head or next to the injector outlet?

Should be a sealing washer on both sides of the banjo fitting.

A U shaped copper washer is on each side of the Bango fitting, but a thin copper washer goes also on the bolt side or on the injector side.
So where does the thin copper washer go?

The U washer is all that's supposed to be there. You need 1 gasket washer on each side of the banjo. You don't need 2 stacked anywhere.
