I have weld on brackets, but I like yours better. Where did you get them?
Here are the pictures of my setup. I plan to use a mechanical caliper off of a Kubota RTV for the driveline brake. For the disk I plan to make my own disk on our water jet machine at work. The reason I used the 1st gen calipers is because I had them from the donor truck for th crew conversion. Hope that helps give you some ideas.

I've been running the Chevy K20 based setup (front rotors and calipers) with driveline parking (fabbed my own set-up) and couldn't be happier. The only issue I've had is needing to keep a closer eye on fluid levels in the master cylinder as the small section for the rears empties much faster from wear than it did with the drums.
Not that I'm aware of (someone else may know). I'm planning a conversion to hydroboost and will use a second gen master which has the undivided plastic tank and so didn't research that option.

You could just swap the entire rear end from an 01.5 or 02 truck. You could use a D70 or a D80 and the factory aluminum wheels will fit too. These rearends are a little wider than 1st gens but with the deeper offset of the factory 16x8 wheels they fit and look like it came from the factory. I used a D80 from an 02 for mine.
I have a Ford 10.5" disc, Ford D70 disc, and a Dodge D80 disc. They all have a different shaped housing flange than the drum brake axles.
Probably less work overall to a do an axle swap unless you really like to cut & splice like this guy:
