Slave cylinder or Master cylinder or both?

Slave cylinder or Master cylinder or both?
Driving to pick kids up yesterday, come to a redlight. Have it in neutral. Light turns green, push clutch in, put in 2nd, and as I am waiting for the cars in front of me to start moving, I notice my truck starts moving forward without me taking my foot off pedal. About that time, cars start moving, I go through gears, everything is fine.
Come to a 4 way stop. Have to stop and go couple times, and then all of a sudden, same thing. With clutch pedal pushed in and waiting, truck starts moving forward without me releasing pedal.
So, I pull into daycare, but now, when I go to push clutch in, it will not disengage, and I can not pull it out of gear, so I have to slam on brakes, and truck stalls (obviously) I get it out of gear (with truck off). Push clutch in, try to put it in gear, and will not go. Just grinding. Obviously clutch is not disengaging at all.
So, slave cylinder? Master cylinder? Or both? I read that once the clutch line is hooked up to teh slave for you NOT to remove it. SO, wouldn't that lead to having to replace BOTH the MC and slave regardless?
Also, fluid level is OK, no leaks either (MC or slave)
Truck:
'03 HO QC LB DRW 2WD NV5600 with 160K miles on it. Bought truck with 60K on it, and I have not replaced anything other than tranny fluid a few times. Cllutch is fine, shifts real smooth all the time, does not slip.
Thanks guys

Maybe I should put this in teh "help" section since my truck is broke down and I got to get back on teh road.....
Either way I need an answer. I mean, I am pretty sure is one or the other, and probaly gonna do both for piece of mind, but COULD it be something else???

I would replace both. They typically come as a sealed assembly. Mine did the same thing and I replaced them with the South Bend parts. These made the tranny shift smoother as well. Very simple to replace. The hardest part is wiggling around on the floorboard trying to unhook the pedal
