How durable is the 48re auto

Just announced:

Exhaust Brakes are now available for Dodge Pickups with Automatic Transmissions.

The Jacobs Exhaust Brake Kit (Part No. 4089924) is now approved by Cummins and Mopar for use on 2006 model year Dodge Ram Pickup Trucks with the 48RE Automatic Transmission . Model Year 2006 trucks will ship to dealer lots in August 05. Note that this is not available for earlier Model Years.

And I just bought an '05 . . . . . oh well . .

but there should be an update coming for the 05 trucks to be compatible with the exhaust brakes. now how about my 03

Get an auto if you want an auto. If it doesn't hold up to your particular driving then you can get it upgraded through various aftermarket companies. Then it will be stonger than any stock auto or 6 speed on the market.
