valve adjustment

I have a couple questions about the valve adjustment.
why did they turn the engine over counter clock wise
why not hook up a starter button.
and could you adj the valves with the engine running and due one cyl at a time
last question if you had a 89 truck how worried would you be about the pin
in the cam falling out

My guess on the CW is to use the ALT. nut ,
The stater button is to hard to stop on just the right spot .
I've done with eng. running on performance gassers , used clips that went on rockers to keep oil from spraying every were .
The pin I'm not familiar with .
