1991 w250 low idle
My truck is non i/c. When start up after has set over night or is cooler outside it will idle low and get white/grey smoke out of exhaust. Runs ok when rev up and seems to go away after just a little bit when revving. I replaced injectors recently seemed to help some. I have different fuel pin and 3200 gov spring. Truck has 250,000-300,000 miles (not sure odometer was not working when i got). As far as i know injection pump has never been rebuilt.
Does injection pump need rebuilt?

Check to make sure your KSB is functioning properly.
Low idle ( and needing to b rebuilt ) is usually a sign after the engine is hot in warmer weather.
Rebuilding the IP is a maintenance thing @s all,of them eventually need it

White smoke is a big clue. Look for an air leak or bad lift pump. That fault will both drop fuel prime when sitting, and cause difficult start and white smoke.
KSB doesn't make much difference above about 40°.
