what should i look out for?
My grand father is buying a 1996 ram3500, 12v, 5speed man, dully 400,000km. Is there any thing that he should look out for?

Just see how the owner took care of it. I wouldn't be too worried about the engine if left stock, but lots of stuff around it can go wrong just like any other truck. KDP and 5th gear nut can cause problems.

there is 0 rust except a little spot on the left front fender.
