A fuel pressure test would answer some questions quickly.
Last time I hooked one up just for a test I used a guage on a line pinned down with my wiper blade so I could see it work on a quick road test.
OFV is my first guess, with lift pump second and finally split boots/loose clamps as my distant third pick.
