Well I replaced the grid heater and installed a new filter. No real change. Today I took off at 10 am and the outside temp was 60 degrees. Truck did the exact same thing in 58 degree weather. Sputtering, popping and smoking white until warm.... Maybe a bad/stuck injector until hot? I am at a loss. I guess I will have to check fuel pressure and go from there....

60*f and it shouldn't be doing that, heater or not. Check that pressure and get back. I don't buy a sticky injector though. I take it that it hasn't done this since the 80hp injectors went in? I had marine injectors in my second 94 - it smoked and popped for a while, but not in 60 degree weather!
