2006 exhaust valve cross over falling off

2006 exhaust valve cross over falling off
I just installed new injectors. After putting it together and fired it off, the 2nd cly exhaust valve cross over fell off. I put it on again and and retorqued it and it still is happening. Do I need to adjust my valve lash? if so what tolerence? It worked fine previous to me working on it. Could it be another issue. Thanx for any input

You need to identify yourself on signature so we know your equipment.
Yes you should adjust valves.
Spec is on plate, left side timing cover, or in service manual.
Is pushrod seated at rocker and in lifter?
