tranny and vibrations help!

im new to diesel trucks and im only 18 and any help is greatly appreciative, alright i have a 04 QC cummins 4x4 3:73 gears its got the 48re tranny i bought it used about 3 months ago for $10k with 145k miles, the guy sold it to me cheap because he didnt know what was wrong with it, but it hangs into gear i think its 2nd gear going up into 3rd gear. it also goes into O/D at 50mph exactly didnt know if tht was normal, it doesnt like to shift no matter how soft or hard you push the gas, it usually shifts if u let off the gas and then push it agian but something is def wrong, also if your going say 60mph and u floor it it jumps up to about 3200 rpms and never shifts just hangs, i dont know what to do because the tranny shops arounud my place charge a hundred dollars just to test drive to see what the problem is and i dont have that $ to throw away because im sure its gonna be pricy to fix the tranny, and i also have a pretty strong vibrations through the gas petal at around 55-63 mph and i have a brand new set of tires mounted and balanced any help would be great thanks!
