Yes a new pilot bearing was installed. My thoughts were maybe the flywheel was turned down too far and the bolts bottomed out before proper tention was put on pressure plate.

In Oregon. No, has 110K on it. But looks perfect. I took apart due to failed pilot bearing, but replaced the entire mess with a south bend kit. My clutch/pressure plate and flywheel looked fantastic though, I was impressed at how well everything looked, except the pilot bearing...

The pressure plate bolts bottoming out before applying pressure would creat the opposite effect I would think..you would see slippage of the clutch, not a problem with dis-engaging. Any noise to it ? Perhaps the heads of the flywheel bolts are catching the clutch disk enough to keep spinning it...Like hooking up on the radial springs..

No noise and if you barely push the clutch while driving it disengages. I have already ordered a flywheel should be here today.

I have heard the pilot bearing is the weak link in the NV5600 clutch, but there is a kit to machine out the pilot bearing hole to use a pilot bearing from a commercial truck clutch.

I believe you can get the roller bearing from Valair and or South Bend. A machine shop will have to mill a larger bore for the new bearing. Both also sell new flywheel's with the HD roller bearing already installed.
