Low End Acceleration Diminished From New

I have changed the fuel filter, air filter, 38,000 miles on 2003, and recently (last 2 months) we have experienced lag, hesitation, until we reach 1500 to 1700 RPM's. Did not happen previous.
Injector Plugging/restrictions? Remedy?
or other?

Please set us in the right investigatory path.

You are all GREAT!
Thanks for Your Help in advance.

sounds like my truck,I have replaced the pressure relief valve and it got a little better, then my inj. pump started leaking so I rebuilt the pump and it got a little better again but still not what it used to be.so recently I got a rail pressure a guage and come to find that the most rail pressure I can get even with my mods is 18000, so this has now led me to the fuel control actuator.

nope stock exhaust, last night I unplugged the fca and started the truck and my pressure shot up to 26000 so I know the pump is fine and everthing up on the fuel rail is fine so now I'm pretty sure it's the fca.
