G56 Bell housing issue

G56 Bell housing issue

HI....I removed my g56 trans to replace the clutch with a Valair stock replacement single disk & flywheel. I had a South bend single disk in there, upon removal cleaning out the bell housing I noticed the clutch fork had wore a groove in one of the star bolts in the release bearing guide tube & made a groove in the bell housing towards the slave cylinder side. Can anybody give some advice to why this would happen??? I also noticed there was no washer behind the pivot ball. Any help would be greatly appreciated.

No washer behind the pivot bar, if there is supposed to be one, would change the angles the fork would move through and could easily be the reason for some odd wear marks.
