Do the springs have to be removed in order to get the shocks out?
For the lower nut, the access hole that faces the rear is large enough for a 3/8" extension but not big enough for a socket - any thoughts?

No need to remove the coil springs. You just remove the shock tower on the top (three small bolts), then remove the bottom bolt out the front. You don't need to hold the bottom nut from turning -- it's spot welded or somehow fastened to keep it from turning. Then you pull the shock out from the top.
I fit an impact socket right in the hole. On the passenger side I pulled the tower with the shock, on the driver's side I didn't have room for that so I loosened the top shock nut, then removed the tower, and pulled the tower off separately.
