During this cold weather (pretty much since I owned the truck) I get blue smoke when I come up off idle.
My start up procedure is this.
Plug in block heater 2 hours before startup (manual notes 1 hour is required)
Cycle heater grid 3 times...10 seconds off between cycles.
Start rig.
LEave idle, leave come up to fast idle (1000rpm) for a while 2-3 minutes.
Bring up off idle until i slowly build up to 1800-2000rpm.
When I get around 1500-1800 I get blue smoke out the exhaust.
Dont use any oil however and I am about due for my 7500 mile oil change.
Have 58000 on the odometer.
How do I affix a fast idle switch to make the rig idle at 1200-1400-1600 rpm with a manual or electronic override? (On my 6.5 chubby you could jump certain pins on the service port.)

yep, normal... here's my cold weather start up sequence
from home:
plug in block heater 3-4 hours before leaving to work [that long since we've been having -20°c weather
]
unplugging heater
start truck up after one grid cycle
clean off snow & ice from windows
get in truck, drive to work
from work:
no plug at work
start truck after one grid cycle [starts up right at 1000rpm]
clean off ice/snow off windows
get in and wait a minute or two while the engine is sitting at 1000rpm idling
start driving home...
there is no need to idle the engine for more than a few minutes... the engine will heat up much quicker with a light load on it.. when it gets real cold out, i get blue smoke while the engine is at 1000rpm idle...
