Transmission Problems!!!!
i have a 99 dually 24V
PROBLEM 1. everytime i start out from a stop it acts like it doesnt want to shift from 1st gear. if i run it hard it really doesnt want to shift. if i take off slow it shifts much better.
PROBLEM 2. when i start out cold, or if i sit at a light for a minute, when i start to go it feels like i am locked into overdrive. the rpms come up good but the truck doesnt go anywhere for 5 seconds then it like kicks in and starts to go.

First problem sounds like a faulty Governor Solenoid in the Valve Body. Accelerate like you would normally (somewhat hard I guess you could say), let off of the go pedal and see if the transmission shifts into 2nd. Then continue on driving like normal (Im assuming).
I'm not exactly sure on the 2nd problem, others will chime in about it though.

Welcome to the site.
Something else to check....
I had the Speed Sensor on the tailshaft housing go out, causing a late 1st to
2nd shift issue, a couple of years ago.
I think the part was only ~$30 and was easy to change from under the truck.
........
On cold starts, I've found that letting the truck run for around 30 seconds with your foot on the brake in neutral
will help get the fluid circulating in the auto tranny quicker.
(The stock Dodge tranny doesn't circulate the ATF while in park.
Your drain-back valve might be bad allowing the fluid to leak back out of the converter.
No big deal if you just allow the ATF to get circulating before driving off)
If it's doing it while sitting at a light, check the fluid level (engine running, parking brake on, in Neutural)

No insult intended but... needs to be checked warm, after driving a bit, and running in neutral... like Ed said.
Otherwise will read higher than actual.
Changing fluid and filter always a good idea if it's been a while.... or truck is new to you.
If not starting from stop in 1st gear, could also be the Govenor Soleniod. Trucks usually start out in second with this problem but many different scenarios have been reported.
