Does it happen even if you double clutch it? Some times when I'm in a hurry, I can get a grinding, so I always try and take it easy when shifting frrom 4th to 5th.

If the fifth gear nut was starting to come off, would this cause the grinding?

You sure you are not moving the shift lever too far to the right and trying to force it into reverse by mistake? I did that a few times when i first got mine.

fifth gear nut isnt a problem, its a nv5600 since its an HO. I would suspect a possible minor lubrication problem holding up the transition or too quick of an attempt at shifting.
Try changing fluid to Redline MTL i had better luck with it than Amsoil for shifting smoothness in the nv5600. Also like i said try slowing down your shift, truck tranny cant force it in. 4th to 5th always grinds in every 6spd ive driving if im too fast on the shift.
1 last thought Could your clutch be getting loose? does it grind or fight you going into any other gears?

all other gears work fine. i bought the truck new. i think i know how to shift by now, i have redline mtl.... probably syncros are gone 113,000 miles

yeah, i think an auto would have been cheaper. but i like a standard.....it seems better than when it was new,,,, but then i`m useing redline mtl

5600 is a super-strong tranny with weak synchros. For the '02 model year, NVG changed the materials used in the synchros to make them harder. 5600's should be driven like trucks, and a high-quality lube like Redline MTL should be used and changed out every 20-25K miles. An Eaton filter from Geno's Garage is extra insurance that has been recommended by the jobber that rebuilds NVG warranty claims.
