engine shaking just above idle

I have a 91.5 Intercooled D250.

I'm trying to diagnose an intermittent engine shaking just above idle that goes away when given heavy throttle. Doesn't always do this, and when behaving, the engine runs really smooth. Also it's been venting way too much oil out the crankcase breather, too much crankcase pressure. Brand new VE pump, lifter pump, and injectors. Engine starts right up and idles perfectly, so compression should be ok. Any ideas? Need to get to the bottom of this...

Low speed miss + Excess blow by = piston problem.
