9 - 11 MPG??? fuel heater assembly
The intake heater may not be what's failing. It could be one of the heater grid relays. I found that one of my relays was sticking causing the heater grid to stay on, heating the intake housing hot enough to make the paint smoke. Just thumoing on the relay made it let go and shut the heater off. I imagine that this would impact milage (hot air intake) and possibly damage the heatergrid/intake gaskets.
Also, I noticed that you are running larger than stock tires. Are you accounting for speedometer/odometer erroe in your calculations?

I believe that the odometer and speedometer use the same source/sensor.

You'll know that the relay is sticking by watching your voltmeter while the truck is running. If the voltage doesn't cycle between 12V and about 15V, just stays at 12V, the heater grid is pulling to voltage down (normal, but it should alternate). The other way to tell is that, after a short warm-up, the grid should stay off and the intake housing (directly atached to the intake manifold) and the housing should cool down. If the relay is stuck, the housing will get really hot and stay that way. You might even hear the contacts in the offending relay buuzzing from the high current. Thump the relay(s), if it goes off after the thump, ithe relay is probably bad and should be replaced.

Your truck was new w/ either one of these tires. Check your door jamb.
225/75-16 (about 29½" Tall)
245/75-16 (about 30½" Tall)
Your tires now:
285/75-16 (about 33" tall)
also after i took it to this fuel place i noticed that the fuel heater assembly was burnt ontop were the wires go into it. and it was done recently also. i call the shop that did the work and he tells me that they go out all the time. that if any fuel gets on it it causes it to short out. well they just changed the fuel filter above it and other work. and well it wasn't like that before i took it there. and u can tell it was fresh as the wires are still copper colored.
do these really go out all the time????

I just wanted to say that fuel is not a conductor of elecricity so therefore if fuel gets on the wires it will not make them short out. Would be happy to hear what others say about this issue.

ok. everytime i fill up i put power service in.
anyway i haven't done anything different to my truck since i did all that work. and i just had to fill my truck up and i checked to see what my milage was. and it came out to 15.0 mpg.
i am still waiting on my parts for the preheater assembly. hopefully that will help me get some better mileage also.
also i was wondering on the checking of the innercooler tubes. what do i need to put in the 4" abs plug. as in for like an air gauge to tell on the pressure and or for fittings?????? i am going to be going up town tomm to get the stuff to work on that tomm.
