How much can I realistically tow with my '99 2500 Cummins?

You'll be able to pull it, stopping it will be the problem.
Upgrade your brakes, or get a trailer brake controller. I would get some 12, or maybe 14 ply tires on the truck. An exhaust brake would be a nice upgrade, but you will need gauges. Good luck!

By the time you read all these posts saying that you need this and you need that to pull a heavy 5ver you'll see why I suggested a travel trailer.

TT tend to only have about 10% to 15% toung weight, so it's lighter on the rear end. A fifth wheel puts most all the weight on the rear end; in fact, my front axel weight doesn't change when I tow my fifth.

I pull a 40" fifth wheel with a 2010 2500 all the time, No Problemo.
Air bags help a lot. Make sure your rear tires are good and keep them aired up to max....load range E is a must.
Make your your brake controller works good and you are good to go.
One more thing, go through your coolant system, check your fan clutch, thermostat, etc. That kind of load will cause serious overheating issues if you have defencts in your cooling system.
