Dual Wheel Conversions

I'm thinking of changing my 91' 250 into a dually. Who sells dual wheel kits? I am going to put a flat bed on it and I don't want the single wheel to be hiding under the bed. Also, what about wheels/bolt patterns?

I used the arrowcraft conversion on two trucks. An 89 and a 93 Dodge. It worked excellent, very high quality stuff. I think the second one I bought which was a full conversion kit(6-wheels, front+rear adapters, lug nuts, rear fenders) was about $1200-$1400 or so, can't remember exactly except it worked great. After I put the stainless simulators on it you couldn't tell it was not factory. 8)
