What year did the big three offer 4 door extended cab 3/4 tons?

Long thread title, pretty much explains it all tho. I know GM had the 3 door, then the four door. I think Ford went to the 4 door straight away with the Superduty in 99, but can't remember for sure. And Dodge had the option in 98, but was only four doors from 99 on. Thanks for any help.

BTW, only the extended cab with the suicide style rear doors, not a crew cab. Again, thanks.

No clue about GM, but you are right about the Dodge and Ford.
