Having a screw loose usually is not a good thing.
Are there supposed to be a guide on the top and bottom? Are those shavings from the cable?

I moved the guard out of the way to get at the cables. Look closely at the second from the right just above the guard. Yes, from the cable.

Are the first, second and third cable also worn?
Are these in a neutral position?
What are the guides made out of, phenolic?
Could there be a harmonic vibration through the airframe causing them to chatter, how much tension are on the cables?
What led you to check the cables, part of a routine inspection?
Looks like you have a great job.

3. Yes, they are the original cable guides.
4. An STC repositioned the cables slightly and eliminated 1 of the guides. The cable tension is stock. The elevator cables are a lot higher than the rest so subject to wear sooner than the others.
5. Because of the STC and a known history of frayed cables because of it, this area is inspected every 100 flight hours even though it is not on the phase inspection or the STC's instructions for continued airworthiness.
6. Sometimes. It's not so great when the seasonal pressure is on, like now.
Permanently correcting the problem is easy. Getting the FAA to except the fix is hard.
