Dealt with this just a few weeks ago. You can remove the top of the tranny from inside the cab and easily see what has happened, the shifter has slipped into the wrong position. Although it is hard to explain you'll figure it out one screwdriver will pop it back. Once you get it back in sync as long as you shift into 5th straight and easy it will stay fixed. If you ever need to limp home 4wd lo will get you there slowly and you will be able to take off in 5th.
Now to the cause of this--- most often the end of the shifter is worn and allows it to go to places it shouldn't go. Good luck finding a new shifter, they aren't available separately and most junkyards don't want to part out a tranny. Ended up costing the owner about $300 to replace the shifter.
The folks at Standard Transmission of Texas told me there is a spring inside the tranny that can break and cause the same problem. That's all I can tell you about that though.

Well you were right, seen the problem - think the solution is shave 20 to 30 off the spacer inserts that go in the top off the tower ,,this will lower it in the selector sleeves and maybe never happen again.

had the same problem with my 01 took it to the dealer and they couldnt find anything wrong other then the fluid needed to be had it happen again and changed the fluid no problems after that. also watch out if you lug it in 5th that tends to cause the problem also.
