Check the relay and the solenoid for the intake heaters, they fail over time and can either stick on or not turn on at all.

All depends on the temp inside your intake manifold. Maximum heater on time is 120 seconds down to just a few seconds. Heaters shut down after the intake temp is over 60F or you are going over 17 mph.

you know what the farmer said when his horse died?
Darn! he never done that before.........
(please excuse the joke, i couldn't resist)
the code indicates a problem with the ECM, can you clear the code and go from there?

Your grid heater solenoids have died on you. They are mounted on the drivers side fenderwell, just below the battery. I don't have the p/n, but I just went through exactly the same thing. Mine got so hot that they melted the insulation on the cables going to the heaters. Just got a 10mm socket and removed the cables at the heaters. Wouldn't start without ether cold, but otherwise started good. Comes with both relays as an assembly on a board, just swap wires. Takes about 20 minutes to change. Good Luck
