P0628 Fuel pump relay circuit low, or fuel lift pump control circuit low.
Hope this helps, not sure what the fix is.

the factory lift pump on an 04 is mounted to the rear of the fuel filter housing. in a hot and vibrating environment, they are known for failing. i am surprised that yours lasted 117000 miles, pretty good. time for an aftermarket pump that is relocated to a framerail near the tank.

fass, bd, glacier, etc.

all much better than the factory pump and location
