It depends, if the high idle will come on I'll let it run 5 to 10 minutes otherwise 30 seconds and I'm on the way. I don't hammer it, but I drive it gently till the temp gauge comes off the lower peg.

I selected the less than 5 minutes, but time to clarify that selection.
I flip on my PacBrake, when the engine temp reaches somewhere around 170*s the brake kicks off, that's when I take off. I still take it easy until the engine reaches full operating temperatures, but the brake kicking off is my starting point. I see that as a nice constant measuring stick to always doing the same startup/warmup, takes out the guesswork of when to go down the road.

Short sleeve shirt weather temps, about a minute.
Cold but not cold enuff to plug in, 3 to 4 minutes
Plug in temps 3-4 minutes.
Houston don't get real cold.

Summer I wait for oil pressure and go.
Winter I let it run on with the hand throttle set for 15+ min when in the - temps and then go. It won't get warm until I drive for a bit anyway.

Even at -24 below zero I never let it run for more then 5 minutes as its a waste of fuel and it will not warm even with it running at 11-1200 RPM's. I also keep the engine speed under 1800 RPM's until the temp gauge goes above 150 degrees.

Running an engine warms more than the engine. It helps warm the trans/ t-case as well. 5 min is not enough for that reason IMHO.
