Sure it did. Right at the top.

Yeah but that was on the 3rd edition of the survey. :
Edit
My e-mail:
Sirs,
After filling completing the survey on three different occasions, I was just starting to wonder if maybe the same person responsible for the transfer pump design on our beloved 5.9 was now working in the marketing/webmaster department. There are several thousand loyal Cummins/Dodge customers out here on our websites who are still asking about their Cummins cab plates. Just wondering when the plates might be sent out or if/when we will get the next notice to fill the survey out again.
Cummins is #1
Your web designer should be as well.

- I am sorry for the snafus in this survey. Short story is that there was 1. a computer bug which garbled surveys and addresses and 2. an address line was left off - so we emailed these people to get address to send the cab plate. I am checking with the people who are fulfilling the cab plate and will be in touch with you to confirm this. We apologize for the delay- you should see your cab plate soon.
Thanks for your business.

Re:Cummins Survey...Go fill it out!
Just recieved this today. Nov 15
Summary: Cummins Customer Survey
Solution:
Thanks for your email Ed.... on the subject, we liked your email the best and immediately forwarded it to the Ram/Cummins account team. Here's the story, be patient:
We have inquired about the Survey and promised Cab/Dash Plates that were promised, and have learned that many customers have not received their plates. Unfortunately, the reason given was that the webpage neglected to provide a field for entering one's mailing address and/or the data in that field got "garbled."
Rather than attempt to separately connect previous emails with addresses provided after the fact, the Dodge Account team has requested that we tell customers to again go to the site and re-enter their contact information. We have been told that the site is now updated to include an "address field."
We apologize for the delay caused by obvious reasons. Some customers have written in with their address and contact info, and we shall forward such; however, to insure receiving a plate we are requesting that you again submit the form and now include your mailing address.
We thank you for your interest in Cummins products. Please let us know if you need assistance in locating the nearest Cummins-authorized Dealer or Distributor Service Provider. For assistance in locating a Service Provider, feel free to use Cummins North America Dealer Locator, which can be found on Cummins website:
http://www.cummins.com/service_locator/index.cfm
Please let us know if you have other questions and if away from your computer or have a time-critical request that needs more urgent attention, feel free to call us toll-free (from North America) at 1-800-DIESELS (343-7357). Customers may also call Cummins switchboard 1-812-377-5000 (worldwide toll call) and ask for the 800-DIESELS Call Center for assistance.
Email direct: powermaster@cummins.com
Regards,

Re:Cummins Survey...Go fill it out!
Same letter I got. Almost to the word. I filled out the survey (this will be the 4th time) and also emailed them my name and address. Hopefully they will get their poop together and get these things out. Sheesh!!
Re:Cummins Survey...Go fill it out!
Geeze, you guys are sure getting excited about something your getting for[glow=red,2,300]FREE!!!!![/glow]

I think I saw something earlier that they weren't going to send out the cab plates until after the promotion ended, it ends Nov 30th according to the web site.
They'll probably get a much better rate by sending them all at once instead of a few each day (bulk rate maybe?)
Think about this,
cab plate-$10 or so
Freight-$5
multiply this by at least 1000 & that's a lot of $$$$$!!
I wouldn't blame them for trying to save a couple of $ on each one by shipping at the same time.
I'm sure they'll eventually get to us.
