My truck has been doing this for awhile now, and I can’t figure out the problem. When casually driving, the entire truck starts to “surge/shake/stutter” when driving about 40 mph. If I downshift to 4th, it mostly clears up. If I hammer down, never does this vibration thing. Under a load, doesn’t do it. Do I have a dying TC, a bearing issue, or something I have not thought of?

If you have an auto trans then you're getting some torque converter shudder at low rpm and downshifting is the correct response.
I find low rpm operation below 1200-1250 rpm can cause that.
