Fron Pinion Yoke Slipping/ratcheting on pinion

I have a 4WD 2002 Dodge with the Dana front differential and I have an issue that I am hoping someone has seen before. The front pinion yoke slips on the pinion shaft. I found the problem to late in the day to be able to get parts and all the HD parts supply are closed on Sunday.

There does not appear to be any extra slop in the pinion movement and I pulled the front drive shaft and the problem got worse. So I pulled the front cover and looked at the gears. The look great and there is no metal shaving of any kind in the case.

So my questions are:
1. have you had this happen to you and if so was it the yoke or the pinion that gave up? I did not have a big enough socket to pull the yoke to inspect the splines but I will tommorow.

2. Have you ever seen a crush sleeve compress for no apparent reason? I am not saying it happened since there is no extra play but I am really curious if you have seen it happen.

Usually splines will wear out because of either loos or unbalanced driveline or loose nut on the shaft. I have seen the drive yoke split before but usually when that happens it throws the drive shaft because the u-joint is broken, dont ask me how I know that,
