My engine stutters/stalls when it is first started. When I start it, the idle speed is low (bounces between 400 to 700) & it feels like the engine is fighting just to run. After 4 or 5 seconds it smooths out & runs normally.

It does not do this all the time, seems like every third time or so after it has been sitting for 6 hours or more.

What should I be looking at? Any recommendations for a 5.9 mechanic in western PA?

Try the simple stuff change fuel filter. Take note how long the lift pump runs when you key on without starting.
