Your code is "Fuel pump relay control circuit low voltage." If you or any one else needs codes you can go to www.allpar.com/fix/computer-codes.html

I am currently dealing with an 0628 on an early 04 here. The lift pump on this one is shot, same symptoms you are describing. This truck has the LP located on the filter canister, and the gentleman believes that stock is best, so we are going to put the updated "in tank" LP system in for him.
ok this morning took fuel filter out and it was clean while the cover was off i turned the key on and the fuel pump was pumping fuel could hear it running and see it pumping put filter back in turned key on waited about 20 seconds an fired off an ran fine. My question is when these pump are about to go will they get hot and quit working then after they cool off start working again? or do they just quit and thats it?
I have seen those pumps quit, cool down and start working again, also a little jolt can make them majically come back to life ( i.e. hitting the pump gently! ) A lot of times I get vehicles towed in, I can hit the tank ( other trucks/cars) and get the pump to run enough to get it in the shop. I'd repalce the pump. When this code sets, it basically is because the pump is either not drawing enough amperage, or too much amperage. The computer monitors that in a round about way.
That code is related to a bad LP. That is the same exact code mine gave me when the LP started acting up at 24,000 miles. If it is under warranty you can get a factory in tank LP installed or opt for the GDP lift pump kit which is what I would do. Going to an aftermarket one after they install the factory in tank LP is a pain to do. You have to install a draw straw in the fuel tank. If you are going to leave it perty much stock the factory in tank LP works just fine.
