2006 5.9 24v serpentine belt comes off

Serpentine belt comes off, any ideas on where to start troubleshooting? This happens often, I keep a spare and have become a pro at replacing this on the go, it's time to find the reason.

Welcome to the forum... Check your belt tensioner pulley and the belt idler pulley. If the bearings are sloppy then it will allow the belt to walk.
