bilstein shock mystery

I installed bilstein shocks on my rig and have had a weird problem. the lower retaining washer for the rubber bushings vanished on the driver's side causing the shock to bang around. the only way it could have gotten off of there is to split in half. i scratched my head and replaced the washer. now it happened on the passenger side. is anybody else familiar with this? somebody is doing something wrong and i hope it ain't me!

Is the stud intact? Have you checked it by raising the truck at he frame? If that's good then sounds like an install issue...
