It attaches on the firewall, pretty much straight up from the fuel filter, then runs to the switch on the top of the transfer case, then from there to the front axle disconnect.

There are a gob of 4 in one plug on top of the transfer case
2 lines near the bottom of transfer case , if your skid plate is still on might be hard to see and these...and are always coming unplugged.
these two hard lines go to the servo on the front axle. I would check these
1st ( the plug to steel under the transfer case)

Yeah, I found that 4-1 plug on top of the t-case bad on my 96. Even though the vacuum was getting down there, the rubber plug was decayed enough to leak the vacuum and my axle would not engage. You can actually buy that little 4 port rubber plug at the dealer. I pulled the individual lines from the old plug and pushed them into the new plug. Worked like a charm.
Oh, the way I diagnosed this was by reaching up and wiggling/squeezing the plug on the t-case, with the engine running and t-case in 4-hi, and feeling the axle disconnect to tell if it was moving (since you can't hear it move with the engine running).

Looks like the right idea ! Totally simple, One problem area rectified !
Price is OK too.............1st time the 4wd does not work because Dogman for got it might be used for a 4wd ( Imagine That ) and installed the vacuum hoses under / beside the T, Case !!
Snow or mud removes the connection and presto no 4WD !

I put the posi lock on mine and what a difference in having the 2 lo for backing trailers. That vacuum set up is pretty weak. When I want 4x4 I know it will always work. Fairly easy to install, would have been nice to have a lift but laying on the ground isnt the end of the world
