Have you tried bumping the starter to see if the lift pump is working? Cycle the key three times and see if it throws any codes.

So you tried just the quick starter bump and didn't hear the lift pump working? sounds like a dead lift pump

Pull some codes!

Failing injector pumps have been known to cause this problem.

If you don't know how on an 02 you can pull codes very easily.

Get a pencil and paper ready
Turn the key on for a second
Turn the key off for a second
Turn the key on for a second
Turn the key off for a second
Turn the key on and leave it on
Watch in the digital odometer window
Keep watching until you see pDone

The ONLY way to make sure you have ALL the codes is to have it scanned with an OBDII code reader.

Hard starts when warm and white smoke when it starts are classic signs of a dead Injector Pump. If thats the case, the codes will be P1688, or P0126. My guess is that both the lift pump AND injector pumps are shot.

Texan, if you dont already have a fuel pressure guage in the cab - its time to get one.
