Lot's of questions, I am not familar with the auto trans truck at all, and only ridden in one, John's "Red Rocket".
Are all auto's 4:10 gear ratio? What kind of service/repairs might he be looking at with 200,000 miles? He will pull a 3H logan horse trailer, would any upgrades be needed? He is looking for a manual trans truck but they are hard to find.

Nope on the 4.10. It may have them, but the vast majority was equipped with 3.54. Not sure about any repairs. The transmissions were reasonably durable at stock power levels, but at 200k, anything could be worn out. Anything could have already been repaired or replaced too.
With an auto, it will also have the smaller Dana 70 differential. (assuming it is a 2500 truck). I would have a good look at it for worn out bearings at that mileage. When the carrier bearings deteriorate on those they like to spin in the housing and cause some damage. With the truck in neutral and the e-brake on, you can remove the rear differential's fill plug and try to pry the carrier side-to-side with a screwdriver to see if there's any end-play in it.
