I thought about building sometihng like that but i wondered the same thing. Or i thought that maybe the extra leverage from the whole 20 inches would actually put too much strain on the trailer breaking the frame..... like on a 5th wheel any thoughts? That would be a lot easier than making an sliding gooseneck

You get the clearance, but you are putting all the weight behind the axle.
You are not moving the truck mountings on these extender units. the weight distribution the truck sees is not changed. The trailer hitch dynamics are changed as evidenced by the substantial gusseting required to maintain adequate load ratings.

I don't, but only because I couldn't find anyone that sold them other than my local RV dealer, who charges a premium for everything. I went with the B & W instead. I would buy one, though, if I could find one at a fair price.
