Exhaust brake for '92 Cummins

Exhaust brake for '92 Cummins
I have a '92 Dodge Cummins w350 5sp. I tow a 12k fifth wheel and would like to install a exhaust brake. Anyone use one of these things and do they work. I am lookin at the BD brake because of the lower end brakeing. Has anyone heard of problems resulting from using one of these brakes?
