98.5 3500 shaking

I've got a 98 24v 3500. When driving at 65+ i get this really bad shake. at times it will stop for a minute and then start back up. it feels like its in the rear but I'm not sure.

Not long ago it caused about a fourth of the outside edge of tread on the drivers side dual to bald. I'm pretty sure it also caused the bearing at the rear transmission to go out and now my rear diff is leaking also.

I've had the tires checked, balanced, rotated and that did nothing.

Any ideas would be greatly appreciated.

From your description is sounds somewhat specific to the left rear with the tire wear. Loose lugs, stripped stud threads, axle bearing or bent axle. Mud packed in between the duals or inside inner dual? Have someone watch or video while you drive, it might show something. I would look a everything from the trans back.
