The only way I was able to get rid of this code was pulling all 6 injectors. I sent them out to be tested and they came back with different variance codes on the injectors. These are the codes that the PCM uses to adjust fueling. I programed these new codes into the PCM and it fixed problem. This was second repair attempt after I had replaced the FPS, which did not fix the problem. The fuel tested good no contamination, and the fuel return from injectors was in spec. Fuel injector tubes were all torqued to spec and fuel limit valve tested good. The fuel return from CP3 pump was borderline so I replaced. Was still under warranty. All and all a difficult code sometimes to diagnose. Cummins stopped using variance codes in 2013 on the 6.7 diesels. The cost for testing 6 injectors was only $300. The cost to replace just 1 injector is $1600.
