Idle hours are everything less than 1000 RPM, or 0 output on APPS. That is how the "idle hours" add up.

Are the idle hours different from the hour meter (6 seconds on the button?) Is the hour meter total hrs?

Idle hours are part of the total hours. The ECM records idle time and time @ % engine load hours.

My is 440 hours with 13,842 miles for a 31mph average. Suburban traffic I suppose. But most of these are towing miles also. You gotta work these engines.
