Fuel Delivery Problem?

My 04 3500 with 124k has been throwing a fuel delivery code for a while now. It will start to loose power at 50-70 mph. I can change my filter and its good for about 2 weeks or 6 tanks of fuel. Today after filling up it looses all power and barley runs while chugging lots of white smoke. Maybe water but no water light is on. What is the deal?

Sounds like an Injector going South or maybe multiple injectors. White smoke is usually a sign of unburned fuel. Check your oil level too!

If it were injectors wouldnt it smoke white all the time or at least a white hayze? Oil is fine. It wont rev past about 1800 before spitting and sputtering or dying out.
