2004 tranny issues

So maybe someone can help!!! 2004 3500 automatic, just had a NEW Chrysler transmission put in 800 miles ago. Since installing it, it has been back to the shop twice. It was shifting really rough, but when under a load it wouldn't shift out of any gears without letting off the throttle then easing back into it, also when towing (not using towhaul) it shifts into OD around 30mph and boggs down the truck. Both times the transmission shop has just "adjusted the bands, and put a new torque converter in both times", but here we are right back where I originally started!!! I would appreciate any ideas... Oh and when I picked it up last time by the time I got home I checked the tranny level and found it was 4quarts low of fluid!!!! No leaks though, I guess they just didn't check it!!
